Patient Cases 

Patient #1 (WT)

31 yo male with history of chronic nasal congestion, mouth breathing, heavy snoring and significant daytime fatigue and mental fog. He also had an anatomically narrow airway secondary to a skeletal malformation causing him sleep disruptive breathing and sleep apnea. He underwent maxillary, mandibular and chin advancement.  

Procedure:

  1. Two piece Le Fort I osteotomy with expansion, impaction, and advancement.

  2. Bilateral sagittal split mandibular osteotomy with advancement and rigid fixation.

  3. Anterior mandibular advancement, 7.0 mm.

Patient#2 (MR)

59 yo female with history of chronic pain, depression, anxiety &TMD. This patient had significant airway obstruction contributing to sleep apnea and clinical symptoms associated with it. She always struggled with her breathing which has affected her ability to exercise.

Procedure:

  1. Le Fort I osteotomy with advancement and inferior repositioning.

  2. Bilateral interpositional bone graft, maxilla BMP, times two kits.

  3. Bilateral sagittal split mandibular osteotomy with advancement.

  4. Advancement genioplasty, 7.0 mm.

 

Patient #3 (ES)

33 yo female had a history of severe chronic pain and nightmares. She felt like she never was truly in a deep sleep. She experienced jaw pain every morning due to her TMJ issues. As of result of her surgery, her airway was expanded from 4mm to 11mm. Post surgery she has not experiences any TMJ pain and is able to enjoy her life with a better nights sleep.

Procedure:

  1. Le Fort with 10.0 mm advancement 1.5 mm anterior impaction.

  2. Bilateral sagittal split mandibular osteotomy.

  3. Advancement genioplasty.

  4. Application and removal of arch bars.
